As our dedicated social worker, Pat has developed an invaluable weekly check-in program that serves as a lifeline for our homebound community members. Understanding that isolation can profoundly impact wellbeing, particularly among LGBTQ+ seniors, Pat makes personal connections through regular phone calls that blend friendly conversation with attentive wellness checks.
